For many individuals, the world is not merely a collection of experiences but a barrage of sensory and social input that the nervous system must constantly negotiate. When we ask what autism is, we are often met with a clinical list of deficits: challenges in social communication, restricted interests, and repetitive behaviors. However, a more professional and empathetic understanding, grounded in current neurobiological research, suggests that autism is a lifelong neurodevelopmental condition characterized by significant and persistent challenges in social-emotional reciprocity and nonverbal communication [StatPearls, 2025]. While diagnostic criteria often focus on what an individual cannot do, the lived reality of autism is frequently defined by a high-intensity experience of the environment that requires a different set of survival tools.

The sensory aspect of autism is perhaps the most significant yet historically overlooked component of the condition. Sensory processing differences are reported in as many as 97% of autistic individuals and are increasingly recognized as a defining feature that shapes perception, cognition, and adaptive behavior [PMC, 2025]. For many, the hum of a refrigerator or the texture of a specific fabric is physically taxing, triggering a state of high alert that can lead to profound exhaustion. We are often taught that resilience means enduring discomfort for the sake of social cohesion, yet for an autistic person, true resilience often lies in the quiet radical act of honoring these physiological needs. This involves moving away from the grief of what we think a life should look like and toward an acceptance of what it actually requires to be sustainable.

This shift toward acceptance is supported by the neurodiversity perspective, which reframes autism not as a broken version of normalcy but as a natural variation in brain function and behavior [Frontiers, 2026]. This model emphasizes that many of the difficulties experienced by autistic individuals are contextual, arising from a mismatch between their internal processing capacities and an environment designed for a neurotypical majority. This is further explained by the double empathy problem, which suggests that social difficulties are a two-way street caused by a breakdown in mutual understanding between different neurotypes [Milton et al., 2022]. Recognizing this allows us to dismantle the guilt that often accompanies the need for accommodations. It is not a failure of character to wear noise-canceling headphones or to opt out of events that promise sensory overload; these are necessary adjustments for neurophysiological health.

Ultimately, understanding autism means recognizing that the autistic brain often prioritizes local details over global patterns, a concept known as weak central coherence [Booth & Happé, 2010]. While this can make certain social nuances more difficult to parse, it often results in a remarkable capacity for deep focus and an impressive attention to detail. By leaning into adaptability and self-compassion, we can move from a state of constant defense to one of intentional and grounded living. Supporting an autistic individual is not about fixing a disorder but about creating an environment that respects their sensory and social boundaries. When we stop trying to force individuals into a mold never designed for them, we create space for a sense of self defined by authenticity rather than performance.

We are often taught from a young age that resilience means enduring discomfort for the sake of social cohesion, yet true resilience often lies in the quiet radical act of honoring our physiological needs rather than suppressing them. This means moving away from the lingering grief of what we think our lives should look like and toward a functional acceptance of what they actually require to be sustainable and healthy.

The sensory aspect of neurodivergence is not a matter of preference, or a lack of discipline, or a sign of fragility, but a fundamental difference in how our brains filter and prioritize the environment. When the environment feels too loud, too bright, or too tactile, the body enters a state of high alert that can lead to profound exhaustion and long-term burnout. Recognizing this is the first step in dismantling the heavy guilt that often accompanies the need for accommodations. It is okay to choose the quiet path, to wear noise-canceling headphones in public, or to opt out of high-stakes social events that promise nothing but sensory overload. These choices are not failures of character or social inadequacies but necessary adjustments for neurophysiological health. When we stop trying to force ourselves into a mold that was never designed for our neurology, we create space for a sense of self that is defined by authenticity rather than constant performance.

This journey toward sensory acceptance often involves a complex process of reframing our expectations of normalcy and success. For many, this looks like doing things differently in ways that might feel unconventional or even uncomfortable at first to those around us. It might involve changing how we work, how we socialize, or how we structure our domestic lives to minimize distress and prevent sensory spikes. While it is natural to feel a sense of loss or isolation when we cannot participate in the world in the same way as the majority, there is a profound and lasting peace to be found in prioritizing the stability of our own nervous system. By leaning into adaptability and practicing a deep level of self-compassion, we can build lives that respect our limitations while celebrating the unique, often vibrant ways we perceive the world. This shift in perspective allows us to move from a state of constant defense and survival to one of intentional, grounded, and meaningful living.

Understanding the sensory landscape of neurodivergence also requires us to acknowledge that our needs may fluctuate based on internal and external stressors. What is manageable one day may be overwhelming the next, and this variability is not a sign of inconsistency but a reflection of a dynamic biological system. Embracing this reality allows us to let go of the rigid standards we often impose upon ourselves in an attempt to appear more typical. Instead of focusing on the frustration of what we cannot change, we can focus on the creative solutions that allow us to engage with the world on our own terms. This might mean advocating for sensory-friendly spaces, seeking out communities that understand these challenges, or simply giving ourselves permission to rest when the world becomes too much. Neurodivergent describes people whose brains process information, senses, emotions, movement, and social cues differently from what’s typically expected. These differences are part of natural human diversity and shape how someone learns, communicates, focuses, and experiences the world. While “neurotypical” is a common reference point, many advocates emphasize that there may be no single “normal”—just a wide range of human brain diversity.

About “the Spectrum”
“Spectrum” is often used to describe variability in autism and sensory differences. While helpful, it can be misunderstood as “everyone is on the spectrum.” A better framing: different traits vary in intensity and combination from person to person; some create meaningful barriers in a world not designed for those nervous systems, while others may not.

Misophonia, often described as an extreme aversion to specific sounds (such as chewing, tapping, or breathing), is a recognized neurological condition that causes intense emotional and physiological distress. For those who live with it, the struggle is real and often debilitating.

However, a popular yet unsupported claim frequently circulates online: that misophonia is somehow correlated with, or even indicative of, higher intelligence (IQ). While this idea might offer a comforting silver lining to those struggling with the condition, a look at the current scientific landscape reveals a clear conclusion: There is no robust scientific proof that misophonia makes people have a higher IQ.

The Reality of Cognitive Function

When researchers study misophonia, they are looking for underlying causes in the brain, not intellectual benefits. The majority of clinical findings point toward challenging cognitive patterns, not cognitive enhancements.

1. Focus on Impairment, Not Superiority

Experimental studies focusing on individuals with misophonia often reveal that when a trigger sound is present, it actually interferes with cognitive performance. The immediate and severe emotional hijacking caused by the trigger sound diverts mental resources, potentially impairing tasks related to:

  • Selective Attention: The ability to focus on one thing while ignoring others.

  • Working Memory: The capacity to hold and manipulate information temporarily.

The distress fundamentally makes it harder, not easier, to think clearly under those circumstances.

2. The Link to Cognitive Inflexibility

Contrary to the idea of a sharper mind, some studies suggest that misophonia is linked to cognitive rigidity or difficulty with set-shifting.

Set-shifting is the ability to easily transition between different tasks or mental states. For individuals with misophonia, the intense, automatic emotional response to a trigger sound is evidence of this rigidity—the brain gets “stuck” in a threat response, making it difficult to modulate the reaction or focus on anything else.

3. Misinterpreting Neural Hyper-Connectivity

The strongest and most consistent scientific finding regarding misophonia is the discovery of altered brain connectivity. Specifically, MRI studies have shown that in people with misophonia, the auditory cortex (which processes sound) is hyper-connected to the ventromedial prefrontal cortex (vmPFC) and the anterior insular cortex (AIC)—areas associated with emotion, threat detection, and motor control.

This discovery explains the fight-or-flight reaction and the sudden, physical urge to stop the sound. The brain essentially routes the sound directly to the body’s threat response center. This is a mechanism for a specific sensory-emotional reflex, not a marker for superior global intelligence.

The Source of the Misunderstanding

Where does the myth come from? It may be a misinterpretation of research into sensory processing:

  • “Leaky” Sensory Gates: Some research into creativity has explored the concept of “leaky” sensory gating—a tendency for the brain to let in more sensory information than average. This trait has been weakly associated with real-world creative achievement. Since misophonia involves an extreme sensitivity to sensory input, the conditions are sometimes conflated. However, being highly sensitive to certain stimuli (misophonia) is not the same as having high sensory tolerance for novel ideas (creativity), and neither is a direct measure of IQ.

Conclusion

Misophonia is a genuine neurological challenge that deserves compassion and targeted therapeutic approaches. While individuals with the condition are, of course, as intelligent as the general population, the claim that their suffering automatically confers a higher IQ is unsupported by evidence.

The initial rollout of Apple’s Liquid Glass interface in OS 26 was met with significant concern from the accessibility and sensory processing communities. The design, characterized by highly transparent elements and “aggressive layout shifts” where background content moved visibly behind translucent bars, was described by some users with sensory disorders as disorienting and dizzying, making the operating system frustrating or unusable.We are now seeing a crucial reversal. With the release of iOS 26.1 beta 4, Apple has implemented a dedicated settings toggle to manage Liquid Glass appearance, which is expected to go live in the upcoming public release.

This new setting, found under Settings > Display and Brightness, allows users to choose between the original Clear (more transparent) look or a new Tinted option. Selecting the Tinted option significantly increases the opacity and contrast of the Liquid Glass elements across the system, including menu bars and Lock Screen notifications.

A Great Improvement, Though Not the Final Word

This addition is a great improvement and a clear sign that Apple has listened to the critical user feedback from the community. By offering a straightforward switch to a more opaque and less visually aggressive interface, the company has directly addressed many of the sensory processing and visual clarity issues that plagued the initial OS 26 experience.

While the consensus among many accessibility advocates is that the true ideal would be to see no aggressive layout shifts or distracting visual movement at all, this transparency toggle immediately makes the operating system more usable for a large number of affected users. It provides a much-needed compromise, balancing Apple’s aesthetic vision with the functional demands of accessibility, allowing users to restore stability and contrast to their interface without having to sacrifice device performance.

There’s no E in CBT for Misophonia.

There’s no E in CBT for Misophonia. Despite the fact that Exposure Response Prevention (ERP) is a prevalent part of Cognitive Behavioral Therapy for numerous disorders such as OCD or Anxiety, this is not the case in CBT-MISO. Why? Because exposure therapy is for conditions that are cognitive, and misophonia is most likely a neurophysiological condition. That core principle guided my reasoning for creating a CBT manual that is completely devoid of exposure. If exposure cured misophonia—we would all be cured as most misophonia sufferers cannot go a day in their lives without being exposed to the sounds that cause this fight-flight-freeze response.

The purpose of this book is to offer practical coping skills for misophonia from a cognitive behavioral perspective that do not include the false promise of treating misophonia like an anxiety disorder—because it isn’t one.

As a clinician, I firmly believe that there is a time and place for exposure therapy—and neither of those involve living with misophonia. As a sufferer of misophonia myself, I take issue with outdated models of cognitive behavioral therapy that are callously applied to misophonia without careful adaption. All of this is what lead me to write Cognitive Behavioral Therapy for Misophonia, which includes 50 worksheets for clinicians or sufferers of misophonia, none of which promote harmful practices like exposure therapy.

While this book doesn’t offer false promises of a treatment or cure, it does offer a practical and realistic perspective for living with misophonia. It is my hope that this book helps clinicians and persons with misophonia understand the condition and view coping with misophonia as a way to adjust and manage the emotions and distress surrounding the triggers, rather than view misophonia as something that can be bullied away with outdated psychological practices.

Shaylynn Hayes-Raymond on behalf of Misophonia International and the International Misophonia Foundation has been working with the accessibility/disability department at Microsoft to advocate for the blinking text cursor (caret |) to respect system settings on Mac and the apple mobile operating system. Windows versions of these apps already honour this program setting. Microsoft has officially made the change to all of their apps on all apple devices.

For many with sensory disorders, misokinesia, autism, ADHD, and more, the blinking text cursor can cause an aversive reaction.

Shaylynn Hayes-Raymond, a dedicated advocate and founding member of the Misophonia Foundation, announced a significant victory for digital accessibility today: Microsoft has removed the blinking caret (cursor) from its applications on Apple products following a year of advocacy and direct communication.

The change resolves a major sensory trigger for millions of individuals living with Misophonia and other sensory processing sensitivities.

“This is a powerful moment that validates the daily struggles of the Misophonia community,” said Shaylynn Hayes-Raymond. “A blinking cursor might seem trivial to most, but for individuals with Misophonia, that constant visual motion causes genuine stress and pain. This change is a huge step toward making technology more inclusive.”

Misophonia is a chronic condition characterized by an involuntary, intense emotional and physiological reaction (often disgust, anxiety, or rage) to specific, soft sounds, such as chewing, breathing, or tapping. The condition impacts individuals and their families worldwide, often leading to social isolation and disruption of daily life.

This advocacy win comes as the International Misophonia Foundation prepares for its upcoming Misophonia Conference. The Foundation is dedicated to supporting critical research and increasing public understanding of the condition.

Imagine sitting in a café when someone nearby coughs, just a soft throat-clear, barely noticed by others. For James, 29, that small sound can feel like a spotlight, igniting panic and rage. His heart pounds, his chest tightens, and a wave of shame washes over him as he scurries for cover. That’s misophonia.

Misophonia (miso = hatred and phonia = sound), literally “hatred of sound”, is marked by intense emotional and physical reactions to specific noises, most often everyday sounds like chewing, coughing, throat-clearing, sniffing, or pen-clicking. A recent study by King’s College London and the University of Oxford estimates that around 18 % of adults in the UK, nearly one in five, experience misophonia at levels that significantly impair their daily lives. Yet the condition remains little known. Many live with it for years before even learning it has a name.

Coughing: A Hidden Trigger
Chewing is often mentioned when people talk about misophonia, but for James, it’s coughing that sets off the spiral. It can be a single, quiet throat-clear across a train carriage, or we’ll be walking through a quiet supermarket aisle when someone behind us lets out a dry, repetitive cough. To most people, it’s nothing. To James, it’s everything.

I’ve seen it happen. His body stiffens. His jaw locks. His eyes flick toward the sound, then away. He goes silent, completely still, but I know what’s happening inside. He has told me before:

“It feels like jaws are clenching down on the left-side of my head, followed by a headache..”

The response is automatic. A feeling of rage pulses through his body. His heart races. And people around him have no idea. They just think he’s tense, or antisocial.

“They assume my mind realises that people can’t help coughing… The symptoms [when someone coughs] are like PTSD and as the sufferer, I don’t have a choice over that.”

Across the room, the man with the cough takes a sip of coffee, relaxed. He doesn’t know he’s ruined James’s morning. He doesn’t know James is already halfway to a meltdown.

That’s the cruelty of misophonia. The trigger is invisible to everyone else, but the impact is huge, and often, deeply isolating. James isn’t overreacting. His nervous system is reacting for him.

From Discovery to Dialogue
Misophonia was first identified in the early 2000s by neuroscientists Pawel and Margaret Jastreboff, who coined the term to describe people experiencing extreme emotional responses, not just irritation, to specific sounds. While the name implies hatred, the reality is one of involuntary distress.

But misophonia is not new. It has existed long before it had a name; countless people have lived in silence, frequently misunderstood or misdiagnosed. Some were told they were anxious, neurotic, or antisocial.

It wasn’t until 2022 that misophonia was proposed in a Delphi review as a discrete disorder by international experts, a milestone that came decades too late for many. This recent recognition underscores just how new, under-researched and under-recognised this condition still is today.

Finding Relief and Strength
James is not alone. Celebrities like Kelly Osbourne, Melanie Lynskey, and Kelly Clarkson have publicly shared their struggles with misophonia, helping to strip away stigma and reassure people like James that they’re not broken. British comedian Seann Walsh, who featured on Strictly Come Dancing, has also spoken out about needing background music at meals to mask chewing sounds, or else feel the panic rise.

Why the 9th of July Matters
World Misophonia Day is more than awareness; it’s validation and support. It’s a day to spotlight stories like James’s, where ordinary sounds to some, like a cough in a café, can unleash extraordinary distress. It’s a call to friends, families, workplaces, and public spaces: be kind, be patient, be quiet.

It’s also a day to push for greater access to therapy, UK-specific research, and better understanding in schools, healthcare, and everyday life. It’s a reminder that misophonia is real.

By sharing these experiences and statistics, we honour those living with misophonia and invite the world to listen, learn, and maybe stay a little quieter on the 9th of July.

OS 26: A Step Backward for Accessibility?

As a misophonia sufferer and accessibility advocate, I was hopeful that the official release of iOS 26 would fix the sensory issues I encountered in the beta. Unfortunately, the full release has not addressed my concerns, and in many ways, the software remains a frustrating and difficult experience for those with sensory processing disorders and other accessibility needs.

In the beta, I noted that the new “Liquid Glass” UI, with its highly transparent menu bars and aggressive layout shifts, felt “jittery and aggressive.” My initial concern was that the inability to turn off or change these visual features would make the new operating system disorienting and dizzying. With the full public release, these issues have not been resolved. The content still shifts behind the translucent menu bars, which can be particularly challenging for people with sensory disorders or attention problems. This has made the experience of using a device with iOS 26 so difficult that I, and others I have spoken with, have felt the need to downgrade to an older, more stable version of the operating system.

While Apple has included options to Reduce Transparency and Reduce Motion, these are not a complete fix. They do not fully eliminate the problematic visual shifts, forcing users to choose between a visually jarring experience and a less performant one. The very features that were supposed to give the operating system a “fluid” feel are the same ones that cause discomfort and frustration for a significant portion of the user base.

The most frustrating part is the apparent lack of consideration for these issues in the final product. While some new accessibility features were added, they fail to make up for the core usability problems introduced by the Liquid Glass design. It’s a disappointment to see Apple, a company that has historically championed accessibility, release a major software update that feels like a step backward for millions of users who rely on visual clarity and a calm interface.
